Driver’s Test Form: · If you are planning to take your written or driving test you must have a C average for the previous semester. There is a form to be filled out by the middle school office that confirms your attendance and grades. You need to request that form at least 24 hours before you need it. It is the student’s responsibility to come back and pick up the completed form.
